CONSENT FORM
RELEASE OF LIABILITY AND SERVICE AGREEMENT
This Release of Liability and Service Agreement (“Agreement”) is entered into by and between the undersigned client (“Client”) and Reiki Master Healing Palm, Elfa Noble,
____________________________________ (“Practitioner”).
Acknowledgement and Assumption of Risk
1. Client acknowledges that services provided are holistic in nature, non-medical, and not a substitute for professional medical advice, diagnosis, or treatment.
2. Client voluntarily elects to receive services and assumes all risks associated therewith.
Disclosure of Health Conditions
3. Client understands the importance of fully disclosing any known allergies, medical conditions, medications, or other relevant health information prior to service.
4. Client agrees to hold harmless Reiki Master Healing Palm, Elfa Noble, and Practitioner for any adverse reaction that may occur as a result of undisclosed health information.
No Refund Policy
5. All payments for services rendered are final. No refunds, exchanges, or credits will be issued once services are provided.
Release of Liability
6. Client, on behalf of themselves, their heirs, executors, administrators, and assigns, hereby waives, releases, and discharges Reiki Master Healing Palm, Elfa Noble, and Practitioner from all claims, liabilities, demands, or causes of action that may arise from participation in services.
Non-Disparagement
7. Client agrees not to publish, post, or communicate negative or disparaging statements in any public forum, including social media, online reviews, or other platforms, regarding Reiki Master Healing Palm, Elfa Noble.
8. Any concerns or issues will be addressed privately and directly between Client and Practitioner.
Governing Law
9. This Agreement shall be governed by and construed in accordance with the laws of the State of California, without regard to its conflict of law principles.
Cancellation & No Show Policy
10. At the time of booking, we require a credit card to keep on file in the event of late cancellations or no-shows. A minimum of 48 hours’ notice is required for appointment cancellations. VIP members must rebook missed appointment within that calendar month. Cancellations made with less than 24 hours’ notice will incur a $50 fee. Cancellations made with less than 8 hours’ notice or failure to show will be charged the full session fee. If a session is rebooked within the same calendar month, the entire session fee may be applied toward retail purchases.
Entire Agreement
11. This Agreement constitutes the entire understanding between the parties and supersedes all prior discussions, representations, or agreements.
12. If any provision is deemed invalid or unenforceable, the remaining provisions shall remain in full force and effect.

Appointment Confirmation
Appointments are confirmed upon payment or deposit, as stated at time of booking.
Cancellation Policy
- A minimum of 24 hours’ notice is required to cancel or reschedule an appointment
- Cancellations made with less than 24 hours’ notice may result in forfeiture of deposit or session fee.
- No-shows will be charged 100% of the scheduled service fee.
Late Arrivals
Late arrival may result in a shortened session to respect subsequent bookings. Full session fees still apply.
Refund Policy
- Services rendered are non-refundable.
- Prepaid packages are non-refundable but may be transferred at the discretion of management.
- Exceptions may be considered in cases of documented medical emergency.
Reikie Master Healing Palms reserves the right to refuse or discontinue services when necessary for safety or professional boundaries.
Liability Waiver – Lymphatic Drainage (California Version)
I understand that lymphatic drainage services provided by Reikie Master Healing Palms are intended for relaxation and general wellness support only.
I acknowledge that:
- The practitioner is not a medical doctor.
- The service is not intended to diagnose, treat, cure, or prevent any disease.
- Results may vary between individuals.
- I have disclosed all relevant medical conditions.

Who Can Benefit?
Lymphatic drainage may be appropriate for:
- Individuals with mild swelling
- Clients recovering from surgery (with medical clearance)
- Those experiencing stress and fluid retention
- People seeking gentle, non-invasive bodywork
Who Should NOT Do Lymphatic Drainage? (Important)
Lymphatic drainage is not appropriate without medical supervision if someone has:
- Active infection
- Fever
- Blood clots (DVT)
- Severe heart failure
- Uncontrolled kidney failure
- Active cancer (unless cleared by oncology team)
Always require medical clearance for:
- Recent surgery
- Chronic medical conditions
- Autoimmune disease flare-ups
